1. Provide compassionate care to patients and their families by actively listening, supporting, and advocating for their needs.
2. Utilize your Master of Social Work degree to assess and identify the social and emotional needs of patients and develop individualized care plans.
3. Conduct thorough and timely psychosocial assessments to identify patients' strengths and challenges and provide appropriate interventions.
4. Collaborate with interdisciplinary team members to ensure the holistic healing of patients and their families.
5. Provide counseling services to patients and their families to address emotional and psychological needs.
6. Utilize crisis intervention techniques to support patients and families during difficult times.
7. Conduct regular follow-ups with patients and families to monitor progress and adjust care plans as needed.
8. Maintain accurate and confidential patient records, documenting all interventions and services provided.
9. Assist patients and families in accessing community resources and support services.
10. Participate in departmental meetings, trainings, and professional development opportunities to enhance skills and knowledge.
11. Adhere to ethical and legal standards in all interactions with patients, families, and colleagues.
12. Collaborate with insurance companies and other third-party payers to ensure coverage for services provided.
13. Maintain a positive and professional demeanor in all interactions with patients, families, and colleagues.
14. Keep updated on current trends and best practices in social work and integrate them into daily practice.
15. Represent AdventHealth in a positive manner and promote the organization's mission and values.
