- Conduct initial assessments of patients' social and emotional needs.
- Develop individualized care plans based on patients' needs and goals.
- Collaborate with interdisciplinary team members to coordinate and implement care plans.
- Provide counseling and support to patients and their families on various issues such as grief, mental health, and financial assistance.
- Connect patients with appropriate community resources and services.
- Maintain accurate and up-to-date documentation of patient interactions and progress.
- Advocate for patients' rights and needs within the healthcare system.
- Participate in discharge planning and ensure smooth transitions for patients from the hospital to their homes or other care facilities.
- Provide crisis intervention and support during emergencies or difficult situations.
- Stay updated on social work practices and techniques through continuing education and training.
- Collaborate with local agencies and organizations to develop and maintain partnerships for the benefit of patients.
- Adhere to ethical and legal standards in all interactions with patients, families, and colleagues.
- Participate in quality improvement initiatives to enhance the delivery of social services at AdventHealth.
- Serve as a resource and mentor to other healthcare professionals on social work best practices.
- Maintain confidentiality and respect the privacy of patients and their families at all times.
Master's Degree In Social Work (Msw): A Fundamental Requirement For A Social Services Practitioner At Adventhealth Is A Master's Degree In Social Work. This Degree Provides Candidates With The Necessary Knowledge, Skills, And Critical Thinking Abilities To Effectively Address The Complex Social Issues Faced By Patients At Adventhealth.
State Licensure: Candidates Must Possess A Valid State License As A Licensed Clinical Social Worker (Lcsw) Or Licensed Social Worker (Lsw). This License Ensures That The Candidate Has Met The Required Standards Of Education And Training, And Is Competent To Provide Clinical Social Work Services.
Experience In A Healthcare Setting: Candidates Should Have Prior Experience Working In A Healthcare Setting, Preferably In A Hospital Or Medical Center. This Experience Allows Candidates To Better Understand The Unique Challenges And Needs Of Patients In A Healthcare Setting, And To Effectively Navigate The Healthcare System.
Knowledge Of Social Service Resources: A Strong Understanding Of Community Resources And Social Service Programs Is Essential For A Social Services Practitioner At Adventhealth. This Knowledge Allows Candidates To Connect Patients With The Appropriate Resources And Support Systems To Address Their Specific Needs.
