Job Description

  1. Provide compassionate and comprehensive social work services to patients and their families.
  2. Conduct initial assessments and develop individualized treatment plans for patients.
  3. Collaborate with interdisciplinary team members to ensure holistic care for patients.
  4. Offer emotional support and counseling to patients and families during difficult times.
  5. Connect patients and families with community resources and support services.
  6. Advocate for patients' rights and access to necessary services.
  7. Maintain accurate and timely documentation of patient interactions and progress.
  8. Stay up-to-date with current social work practices and regulations.
  9. Participate in team meetings and contribute to treatment planning discussions.
  10. Continuously evaluate and adjust treatment plans to meet patients' evolving needs.
  11. Provide crisis intervention and support during emergency situations.
  12. Act as a liaison between patients, families, and healthcare providers.
  13. Educate patients and families on coping strategies and self-care techniques.
  14. Maintain confidentiality and adhere to ethical standards in all interactions.
  15. Collaborate with community organizations and agencies to coordinate services for patients.
  16. Participate in quality improvement initiatives to enhance patient care.
  17. Attend trainings and conferences to enhance professional development.
  18. Support the mission and values of AdventHealth through compassionate and ethical practices.
  19. Demonstrate a commitment to cultural sensitivity and diversity in all interactions.
  20. Perform any other duties as assigned by the supervisor.

Job Qualifications

  • Master's Degree In Social Work (Msw): The Ideal Candidate For This Role Should Have A Master's Degree In Social Work From An Accredited Institution. This Degree Provides The Necessary Knowledge And Skills To Work Effectively In The Field Of Social Services.

  • State Licensure: The Candidate Should Possess A Valid State License To Practice Social Work. This Ensures That The Candidate Has Met The Requirements And Standards Set By The State To Provide Social Services.

  • Experience In A Healthcare Setting: Adventhealth Is A Healthcare Organization, So Having Experience In A Similar Setting Would Be Beneficial. The Candidate Should Have Experience Working With Patients And Families, Understanding The Healthcare System, And Collaborating With Other Healthcare Professionals.

  • Knowledge Of Social Service Resources: A Social Services Practitioner Should Have A Thorough Understanding Of Available Social Service Resources In The Community. This Includes Knowledge Of Government Programs, Non-Profit Organizations, And Other Resources That Can Benefit Patients And Families.

  • Strong Communication And Empathy Skills: Effective Communication And Empathy Are Essential Qualities For A Social Services Practitioner. The Candidate Should Be Able To Listen Actively, Communicate Effectively, And Show Empathy Towards Patients And Families Who May Be Going Through Difficult Times. These Skills Are Crucial In Building Trust And Rapport With Clients.
