- Develop and implement individualized care plans for patients, in collaboration with the healthcare team.
- Work closely with patients and their families to ensure they receive the appropriate level of care and support.
- Conduct thorough assessments of patients' medical history, current condition, and ongoing needs.
- Monitor and evaluate patients' progress and update care plans as needed.
- Coordinate with other healthcare providers and agencies to ensure seamless care for patients.
- Educate patients and their families on self-management techniques and resources available to them.
- Communicate effectively and compassionately with patients, families, and the healthcare team.
- Serve as an advocate for patients, ensuring their voices are heard and their needs are met.
- Maintain accurate and up-to-date documentation of patient care and progress.
- Adhere to all relevant policies, procedures, and regulations, including those related to patient confidentiality.
- Participate in ongoing education and training to stay current on best practices and advancements in healthcare.
- Collaborate with interdisciplinary team members to identify and implement quality improvement initiatives.
- Act as a resource and mentor for other healthcare team members, including nurses and support staff.
- Remain up-to-date on insurance and reimbursement policies and assist with insurance-related issues.

Bachelor's Degree In Nursing: A Minimum Of A Bachelor's Degree In Nursing Is Required For A Rn, Care Manager Position At Adventhealth. This Ensures That The Candidate Has A Strong Foundation In Nursing Practice And Theory.
Current Rn License: A Valid And Current Rn License Is A Must-Have Qualification For This Role. The Candidate Should Be Registered With The State Nursing Board And Must Maintain A Good Standing.
Minimum Of 2 Years Of Clinical Experience: Adventhealth Requires A Minimum Of 2 Years Of Clinical Experience As A Registered Nurse For Their Rn, Care Manager Role. This Ensures That The Candidate Has A Strong Understanding Of Patient Care And Can Effectively Manage A Team Of Nurses.
Case Management Certification: A Certification In Case Management, Such As The Certified Case Manager (Ccm) Or Accredited Case Manager (Acm), Is Highly Preferred By Adventhealth For Their Rn, Care Manager Position. This Certification Showcases The Candidate's Knowledge And Skills In Care Coordination And Management.
Excellent Communication And Leadership Skills: As A Care Manager, The Candidate Will Be Responsible For Leading A Team Of Nurses And Coordinating Care For Patients.
