  1. Coordinate and manage patient care: The main responsibility of the Care Manager will be to oversee and coordinate the care of patients, ensuring that they receive the best possible treatment and support.
  2. Advocate for patient needs: The Care Manager will act as an advocate for patients, ensuring that their needs and preferences are considered in their care plan.

  7. Educate patients and families: The Care Manager will provide education to patients and their families about their conditions, treatment options, and self-care techniques.
  8. Ensure quality and safety: The Care Manager will adhere to all safety and quality standards, ensuring that patients receive safe and effective care.
  9. Maintain accurate records: The Care Manager will document all patient care activities accurately and in a timely manner, following all relevant policies and procedures.
  10. Demonstrate empathy and compassion: As a representative of AdventHealth's mission, the Care Manager must demonstrate empathy and compassion towards patients and their families, creating a supportive and healing environment.
  11. Stay updated on healthcare advancements: The Care Manager will stay updated on the latest healthcare advancements and best practices to provide the most effective care possible.
  12. Adhere to ethical and legal standards: The Care Manager must adhere to all ethical and legal standards in patient care and maintain patient confidentiality at all times.

Job Qualifications
  • Bachelor's Degree In Nursing From An Accredited Institution.

  • Current And Unrestricted Registered Nurse (Rn) License In The State Of Employment.

  • Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In Care Management Or Case Management.

  • Knowledge And Understanding Of Healthcare Regulations, Including Medicare And Medicaid.

  • Strong Communication, Organization, And Critical Thinking Skills.
