1. Provide compassionate and personalized care to patients in their homes, following the specific treatment plans prescribed by the Physical Therapist.
2. Conduct initial patient assessments and develop individualized treatment plans based on patient needs and goals.
3. Work collaboratively with the Physical Therapist and other healthcare professionals to ensure coordinated and effective care for patients.
4. Implement therapeutic exercises, activities, and modalities to help patients improve their mobility, strength, and overall function.
5. Educate patients and their families on proper techniques and exercises to continue their progress between therapy sessions.
6. Continuously monitor and document patient progress, adjusting treatment plans as needed.
7. Maintain accurate and thorough patient records, including progress notes, evaluations, and discharge summaries.
8. Maintain a safe and clean treatment environment, adhering to infection control and safety protocols.
9. Communicate effectively with patients, families, and healthcare team members to ensure optimal care and outcomes.
10. Stay up-to-date with current physical therapy techniques and research, and incorporate new knowledge into patient treatment plans.
11. Adhere to all ethical and legal standards for patient care and confidentiality.
12. Participate in quality improvement initiatives and provide feedback for program improvement.
13. Represent AdventHealth and its mission of compassionate care in all interactions with patients, families, and colleagues.
14. Other duties as assigned by the Physical Therapist or supervisor.
