- Maintain accurate and up-to-date records of patient information and treatment plans.
- Provide guidance and support to patients and their families, addressing any questions or concerns they may have.
- Communicate with healthcare providers to ensure seamless coordination of patient care.
- Monitor and track patient progress and follow up on any missed appointments or treatments.
- Collaborate with other healthcare professionals to develop and implement patient care plans.
- Educate patients on health management and self-care techniques.

Experience In Healthcare Setting: A Strong Understanding And Experience Working In A Healthcare Setting Is Essential For A Patient Care Coordinator At Adventhealth. This Includes Knowledge Of Medical Terminology, Procedures, And Regulations.
Excellent Communication Skills: Patient Care Coordinators At Adventhealth Must Have Exceptional Communication Skills To Effectively Communicate With Patients, Families, And Healthcare Professionals. This Includes Both Written And Verbal Communication Skills.
