- Provide personal care to patients in their homes, including assistance with activities of daily living such as bathing, dressing, and grooming.
- Follow care plans and instructions from healthcare professionals to ensure that patients' needs are met and their health and safety are maintained.
- Monitor and report any changes in patients' condition or behavior to the appropriate healthcare professionals.
- Assist with mobility and transfers, using proper body mechanics and equipment as needed.
- Help patients with medication reminders and medication administration, under the direction of a licensed healthcare professional.
- Maintain a clean and safe environment for patients, including light housekeeping duties.
- Promote and maintain patients' independence, dignity, and privacy at all times.
- Communicate effectively with patients, their families, and other healthcare team members to provide quality care and ensure patients' needs are met.
- Demonstrate empathy, compassion, and a positive attitude towards patients and their families.
- Participate in ongoing training and education to stay updated on best practices and provide the highest level of care.
- Adhere to all company policies, procedures, and protocols to ensure compliance with healthcare regulations and standards.
- Maintain accurate and timely documentation of patient care and activities.
- Collaborate with other healthcare professionals to develop and implement individualized care plans for patients.
- Be an advocate for patients and their needs, while also adhering to professional boundaries and ethical standards.
- Continuously strive for excellence in providing compassionate and high-quality care to improve patients' overall well-being.
Current Cna Certification: Adventhealth Requires All Home Health Aides And Cnas To Have A Valid And Up-To-Date Certification From An Accredited Program. This Ensures That The Candidate Has Completed The Necessary Training To Provide Quality Care To Patients In A Home Health Setting.
Experience In Home Health Care: Prior Experience In A Home Health Care Setting Is Preferred For This Role. Candidates Should Have A Strong Understanding Of The Unique Challenges And Responsibilities Involved In Providing Care In A Patient's Home.
Compassion And Empathy: As A Home Health Aide/Cna, It Is Essential To Have A Compassionate And Empathetic Approach To Patient Care. Candidates Should Be Able To Demonstrate A Genuine Interest In Helping Others And Providing Support In A Home Setting.
