1. Provide compassionate and personalized care to patients in their homes.
2. Perform comprehensive assessments and develop individualized care plans.
3. Administer medications and treatments as prescribed by physicians.
4. Monitor and evaluate patient progress and communicate any changes to the healthcare team.
5. Educate patients and their families on proper care techniques and disease management.
6. Collaborate with interdisciplinary team members to ensure quality and coordinated care.
7. Maintain accurate and timely documentation of patient care.
