  1. Provide quality in-home nursing care to patients with life-limiting illnesses
  2. Support and educate patients, families, and caregivers on hospice care and end-of-life issues
  3. Perform comprehensive assessments and develop individualized care plans for patients
  4. Administer medications and treatments as prescribed by physicians
  5. Monitor and document patient progress and response to treatments
  6. Collaborate with interdisciplinary team members to provide holistic care for patients
  7. Ensure compliance with all state and federal regulations and agency policies
  8. Maintain accurate and up-to-date patient records
  9. Provide emotional support and comfort to patients and their families during difficult times
  10. Participate in on-call rotation and respond promptly to patient needs
  11. Continuously seek opportunities for professional development and education

Job Qualifications
  • Valid Lvn License: The First And Most Important Qualification For A Field Lvn (Hospice) Job At Adventhealth Is A Valid Lvn License In The State In Which The Job Is Located. This License Ensures That The Candidate Has Completed The Necessary Education And Training To Provide Nursing Care In A Hospice Setting.

  • Hospice Experience: Adventhealth Prefers Candidates With At Least 1-2 Years Of Experience Working In A Hospice Setting. This Experience Demonstrates The Candidate's Understanding Of The Unique Needs And Challenges Of Hospice Patients And Families.

  • Excellent Communication Skills: A Field Lvn (Hospice) At Adventhealth Must Have Excellent Communication Skills To Effectively Communicate With Patients, Families, And The Interdisciplinary Team. This Includes The Ability To Listen Actively, Provide Compassionate Care, And Communicate Effectively With Individuals From Diverse Backgrounds.

  • Strong Clinical Skills: As A Field Lvn, The Candidate Must Possess Strong Clinical Skills To Provide Quality Nursing Care To Hospice Patients. This Includes The Ability To Assess And Monitor Patients, Administer Medications, Perform Wound Care, And Provide Comfort Measures To Patients.

  • Compassionate And Empathetic: Working In Hospice Requires A High Level Of Compassion And Empathy. Candidates For A Field Lvn (Hospice) Job At Adventhealth Must Possess These Qualities To Provide Holistic Care To Patients And Support Their Families During End-Of-Life Care.
