Job Description

In this role, you will provide compassionate and quality care to patients in their homes, helping them and their families navigate through the end-of-life journey.   1. Provide compassionate and quality care to hospice patients in their homes.
  2. Assist patients and their families in navigating the end-of-life journey.
  3. Collaborate with the hospice team to create and implement individualized care plans.
  4. Monitor and assess patients' physical, emotional, and spiritual well-being.
  5. Administer medications, treatments, and procedures as prescribed by the physician.
  6. Educate patients and families on disease progression and symptom management.
  7. Maintain accurate and thorough patient records.
  8. Communicate effectively with patients, families, and other healthcare team members.
  9. Adhere to all hospice policies, procedures, and regulatory requirements.
  10. Continuously evaluate and improve the quality of care provided.
  11. Serve as a patient advocate and ensure their needs and wishes are honored.
  12. Provide emotional support and comfort to patients and their loved ones.
  13. Collaborate with community resources to ensure continuity of care.
  14. Attend and actively participate in team meetings and educational opportunities.
  15. Maintain a safe and clean work environment.

Job Qualifications
  • Valid Lpn License: The Candidate Should Possess A Valid And Active License As A Licensed Practical Nurse In The State Where Adventhealth Is Located.

  • Hospice Experience: Previous Experience In Hospice Care Is Preferred For This Role. The Candidate Should Have A Good Understanding Of End-Of-Life Care And Be Comfortable Working With Patients And Families During This Difficult Time.

  • Strong Communication Skills: As A Field Lpn, Hospice, Effective Communication With Patients, Families, And Healthcare Team Members Is Crucial. The Candidate Should Be Able To Communicate Clearly And Compassionately In Both Verbal And Written Forms.

  • Compassionate And Empathetic: Providing Care To Patients In Hospice Requires A High Level Of Compassion And Empathy. The Candidate Should Have A Kind And Caring Demeanor And Be Able To Support Patients And Families Through Emotional And Physical Challenges.

  • Ability To Work Independently: This Role Involves Working In The Field, Often Traveling Between Different Patient Homes. The Candidate Should Be Able To Work Independently And Manage Their Time Effectively To Provide Quality Care To Patients In Various Locations.
