  1. Develop and implement health education programs and initiatives that align with the needs of the community.
  2. Connect community members with appropriate healthcare resources and services.
  3. Advocate for improved access to healthcare services for underserved populations.
  4. Build and maintain relationships with community organizations, leaders, and stakeholders to promote collaboration and support for health initiatives.
  5. Conduct outreach and engage with community members to assess their needs and identify potential health concerns.
  6. Provide culturally competent support and resources to individuals and families from diverse backgrounds.
  7. Collaborate with healthcare professionals to ensure continuity of care for community members.
  8. Document and maintain accurate records of community interactions and interventions.
  9. Monitor and evaluate the effectiveness of health education programs and initiatives.
  10. Continuously seek new opportunities for community partnerships and collaborations to enhance the impact of health initiatives.
  11. Stay up-to-date on current health trends and best practices in community health.
  12. Attend relevant trainings and conferences to enhance skills and knowledge in community health work.
  13. Adhere to ethical standards and maintain confidentiality in all interactions with community members.
  14. Participate in team meetings and contribute ideas for improving community health outcomes.
  15. Represent AdventHealth in a professional and positive manner while engaging with the community.

Job Qualifications

  • Knowledge Of Public Health Principles: Community Health Workers At Adventhealth Should Possess A Strong Understanding Of Public Health Principles And Be Able To Apply Them In Their Work. This Includes Knowledge Of Disease Prevention, Health Promotion, And Community Health Education.

  • Excellent Communication Skills: Effective Communication Is A Crucial Part Of A Community Health Worker's Job At Adventhealth. They Should Be Able To Communicate Clearly And Effectively With Individuals From Diverse Backgrounds, Including Patients, Families, And Healthcare Providers.

  • Cultural Competency: Adventhealth Is Committed To Providing Culturally Sensitive Healthcare Services. As Such, Community Health Workers Should Be Culturally Competent And Able To Work With Diverse Populations To Understand Their Unique Health Needs And Provide Appropriate Support.

  • Experience In Community Outreach: Community Health Workers At Adventhealth Should Have Experience In Community Outreach And Be Able To Engage With Community Members To Promote Health And Wellness Initiatives. They Should Also Have A Strong Understanding Of Community Resources And Be Able To Connect Individuals To Appropriate Services.

  • Empathy And Compassion: At Adventhealth, We Believe In Treating Each Patient With Compassion And Empathy. Community Health Workers Should Possess These Qualities And Be Able To Provide Emotional Support To Individuals In Need.
