- Serve as a liaison between the healthcare system and the community, building relationships and promoting collaboration.
- Identify and assess the healthcare needs of the community, particularly those who are underserved or marginalized.
- Develop and implement strategies to improve access to healthcare services and resources for community members.
- Conduct outreach and education programs to raise awareness about health and wellness and empower individuals to take control of their own health.
- Provide culturally sensitive support and advocacy for individuals and families in need of healthcare services.
- Collaborate with healthcare providers to coordinate care and ensure that patients receive appropriate and timely services.
- Maintain accurate and up-to-date records of community interactions and referrals made.
- Participate in community events and initiatives to promote health and wellness.
- Stay informed about community resources and services and make appropriate referrals as needed.
- Advocate for policy changes and improvements to address the social determinants of health and promote health equity.
- Provide emotional support and crisis intervention to community members in need.
- Maintain confidentiality and adhere to ethical standards in all interactions with community members.
- Attend trainings and conferences to stay updated on best practices and trends in community health.
- Collaborate with other community health workers and professionals to share knowledge and resources.
- Continuously evaluate and assess the impact of community health programs and make recommendations for improvement.
Bachelor's Degree In Public Health, Social Work, Or A Related Field.
Certification As A Community Health Worker Or Equivalent Training.
Minimum Of 2 Years Experience Working In A Community Health Setting.
Bilingual Proficiency In English And Spanish.
Strong Communication And Interpersonal Skills, With The Ability To Work With Diverse Populations.
