- Review and analyze patient medical records to ensure accuracy and completeness of documentation.
- Collaborate with healthcare providers to identify and clarify missing, conflicting, or inaccurate information in patient records.
- Conduct thorough research and investigation to resolve documentation discrepancies.
- Educate healthcare providers on proper documentation practices and provide ongoing support and guidance.
- Utilize coding and classification systems to accurately capture and report patient diagnoses and procedures.
- Monitor and report on documentation trends and identify areas for improvement.
- Stay up-to-date on industry changes and regulations related to clinical documentation integrity.
- Maintain a high level of attention to detail and accuracy while working in a fast-paced and dynamic environment.
- Communicate effectively and professionally with interdisciplinary teams and external stakeholders.
- Participate in audits and quality improvement initiatives to ensure compliance and quality of patient records.
- Collaborate with billing and coding teams to ensure accurate and timely reimbursement for services provided.
- Continuously seek opportunities for professional development and growth to enhance knowledge and skills.
- Adhere to all hospital and departmental policies and procedures.
- Maintain patient confidentiality and comply with HIPAA regulations at all times.

Bachelor's Degree In Nursing Or Related Healthcare Field
Minimum Of 3 Years Experience In Clinical Documentation Integrity Or Related Role
Knowledge Of Healthcare Coding Systems (Icd-10, Cpt, Hcpcs)
Strong Understanding Of Medical Terminology And Disease Processes
Certification In Clinical Documentation Improvement (Cdip) Or Certified Coding Specialist (Ccs) Preferred
