Job Description

Willis-Knighton Health System is seeking an experienced and caring Discharge Planner to join our team. As a Discharge Planner, you will be responsible for ensuring that our patients have a smooth and successful transition from hospital to home and the necessary resources to support their health and well-being. This role requires an individual with a passion for making a positive difference in the lives of our patients and their families.To be successful in this position, you must have an in-depth understanding of the healthcare system and the resources available to our patients, excellent interpersonal communication and organizational skills, and the ability to work well with a variety of people. The ideal candidate will have a Bachelor’s degree in Health Administration or a related field, a minimum of two years’ experience as a Discharge Planner, and be comfortable working with a diverse patient population. The ability to think outside the box, work independently and as part of a team, and exhibit a commitment to patient-centered care are also essential for this role.

Responsibilities:

  1. Ensure that patients have a successful transition from hospital to home.
  2. Identify and assess patients’ needs and resources within the healthcare system.
  3. Develop and implement discharge plans that meet the individual needs of each patient.
  4. Facilitate interdisciplinary collaboration to ensure continuity of care.
  5. Establish relationships with community resources to support patient care.
  6. Monitor and evaluate patient progress and modify discharge plans as needed.
  7. Maintain accurate documentation of patient discharge plans and progress.
  8. Provide education and support to patients and their families.
  9. Remain up-to-date on the latest advances in healthcare and discharge planning.
  10. Work with other healthcare professionals to ensure the highest quality of care for patients.
