
Discharge Planner
Willis-Knighton Health System is seeking an experienced and caring Discharge Planner to join our team. As a Discharge Planner, you will be responsible for ensuring that our patients have a smooth and successful transition from hospital to home and the necessary resources to support their health and well-being. This role requires an individual with a passion for making a positive difference in the lives of our patients and their families.To be successful in this position, you must have an in-depth understanding of the healthcare system and the resources available to our patients, excellent interpersonal communication and organizational skills, and the ability to work well with a variety of people. The ideal candidate will have a Bachelor’s degree in Health Administration or a related field, a minimum of two years’ experience as a Discharge Planner, and be comfortable working with a diverse patient population. The ability to think outside the box, work independently and as part of a team, and exhibit a commitment to patient-centered care are also essential for this role.
Responsibilities:
- Ensure that patients have a successful transition from hospital to home.
- Identify and assess patients’ needs and resources within the healthcare system.
- Develop and implement discharge plans that meet the individual needs of each patient.
- Facilitate interdisciplinary collaboration to ensure continuity of care.
- Establish relationships with community resources to support patient care.
- Monitor and evaluate patient progress and modify discharge plans as needed.
- Maintain accurate documentation of patient discharge plans and progress.
- Provide education and support to patients and their families.
- Remain up-to-date on the latest advances in healthcare and discharge planning.
- Work with other healthcare professionals to ensure the highest quality of care for patients.
