  1. Provide support and resources to individuals struggling with addiction.
  2. Collaborate with other healthcare professionals to create individualized treatment plans.
  3. Conduct thorough assessments to determine the needs of each patient.
  4. Develop and implement interventions and strategies to assist patients in their recovery journey.
  5. Advocate for patients and connect them with community resources and support systems.
  6. Educate patients and their families on addiction, treatment options, and the recovery process.
  7. Monitor and evaluate patient progress and adjust treatment plans accordingly.
  8. Maintain accurate and confidential records of patient interactions and progress.
  9. Attend regular team meetings and participate in case conferences to discuss patient care.
  10. Stay up-to-date on the latest research and best practices in addiction treatment.
  11. Maintain a strong understanding of ethical and legal standards in social work practice.
  12. Work closely with insurance companies and other funding sources to ensure coverage for treatment.
  13. Provide crisis intervention and support for patients in distress.
  14. Serve as a liaison between patients, families, and treatment facilities.
  15. Continuously assess and improve the effectiveness of the addiction program at the hospital.

Job Qualifications
  • Master's Degree In Social Work From An Accredited Institution.

  • Current Licensure As A Licensed Clinical Social Worker (Lcsw) In The State Of New Hampshire.

  • Minimum Of 2 Years Of Experience Working With Individuals Struggling With Substance Use Disorders.

  • Knowledge And Understanding Of Evidence-Based Treatment Modalities For Addiction, Including Motivational Interviewing And Cognitive Behavioral Therapy.

  • Ability To Work Collaboratively With A Multidisciplinary Team And Provide Comprehensive Support And Care To Patients With Co-Occurring Mental Health And Substance Use Disorders.
