  1. Conduct comprehensive assessments of patients and their families to determine their emotional, social, and practical needs during the pregnancy and childbirth process.
  2. Collaborate with a multidisciplinary team to develop and implement individualized care plans for patients and their families.
  3. Provide emotional support, counseling, and education to patients and their families to help them cope with the challenges of a high-risk pregnancy and fetal care.
  4. Advocate for patients and their families to ensure their needs are met and their voices are heard within the healthcare system.
  5. Develop and maintain relationships with community resources to provide additional support and assistance to patients and their families.
  6. Facilitate support groups and educational programs for patients and their families to promote emotional well-being and self-care.
  7. Document all patient interactions and interventions accurately and in a timely manner.
  8. Stay up-to-date on current research, best practices, and legislation related to fetal care and social work.
  9. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
  10. Participate in quality improvement initiatives to enhance the delivery of care to patients and their families.
  11. Provide crisis intervention and risk assessments as needed.
  12. Collaborate with insurance companies and other agencies to secure necessary resources for patients and their families.
  13. Serve as a liaison between patients, families, and the healthcare team to promote effective communication and continuity of care.
  14. Attend and actively participate in team meetings, trainings, and other hospital initiatives.
  15. Uphold the mission, values, and standards of Wentworth-Douglass Hospital in all aspects of the role.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited Institution.

  • Minimum Of 2 Years Of Experience Working With Pregnant Women And Families In A Healthcare Setting.

  • Knowledge Of Prenatal Care Guidelines And Resources For Pregnant Women.

  • Excellent Communication And Interpersonal Skills To Work With Diverse Populations And Collaborate With A Multidisciplinary Team.

  • Strong Understanding Of Ethical Principles And Ability To Maintain Confidentiality In All Aspects Of The Job.
