  1. Assess and evaluate the health and wellness needs of the local population.
  2. Develop and implement population health programs and initiatives to address identified needs.
  3. Collaborate with interdisciplinary teams to identify and address social determinants of health that impact the community.
  4. Conduct outreach and engage with community members to promote health education and disease prevention.
  5. Provide direct patient care, including screenings, vaccinations, and health coaching.
  6. Monitor and track health outcomes and identify areas for improvement.
  7. Serve as a resource and advocate for patients, families, and caregivers.
  8. Collaborate with external stakeholders, such as community organizations and public health agencies, to address population health needs.
  9. Document and maintain accurate records of patient interactions and program outcomes.
  10. Stay up-to-date on current healthcare trends, research, and evidence-based practices related to population health.
  11. Participate in quality improvement initiatives to enhance the delivery of population health services.
  12. Demonstrate strong communication skills to effectively educate and engage patients and the community about health and wellness.
  13. Adhere to ethical and legal standards of nursing practice.
  14. Work collaboratively with other healthcare professionals to provide comprehensive care to patients.
  15. Continuously evaluate and adapt population health strategies to meet the changing needs of the community.
  16. Maintain confidentiality and respect the privacy of patients and their families.
  17. Attend training and professional development opportunities to enhance knowledge and skills related to population health.

Job Qualifications
  • Bachelor's Degree In Nursing From An Accredited Institution: A Population Health Nurse At Wentworth-Douglass Hospital Must Possess A Bachelor's Degree In Nursing To Ensure That They Have A Strong Foundation In Healthcare Principles And Practices.

  • Active Rn License In The State Of New Hampshire: In Order To Practice As A Nurse In The State Of New Hampshire, The Candidate Must Have An Active Rn License. This Ensures That They Have Met All The Necessary Requirements To Provide Safe And Effective Patient Care.

  • Minimum Of 2 Years Of Experience In Population Health Or Community Health Nursing: Candidates For This Role Should Have A Minimum Of 2 Years Of Experience In Population Health Or Community Health Nursing. This Experience Helps Them To Understand The Unique Needs Of The Community And Develop Effective Strategies To Improve Overall Health Outcomes.
