  1. Provide virtual patient care to improve patient outcomes and support the growing community of Wentworth-Douglass Hospital.
  2. Utilize strong clinical skills and knowledge to assess and treat patients in a virtual setting.
  3. Communicate effectively with patients, colleagues, and other healthcare professionals to coordinate care and ensure quality outcomes.
  4. Collaborate with the healthcare team to develop and implement virtual care plans for patients.
  5. Remain up-to-date on the latest healthcare technology and tools used for virtual patient care.
  6. Adhere to all HIPAA regulations and maintain patient confidentiality at all times.
  7. Serve as a resource for patients and their families, providing education and support as needed.
  8. Monitor patient progress and make necessary adjustments to treatment plans as needed.
  9. Document all patient interactions, treatments, and progress accurately and in a timely manner.
  10. Participate in ongoing training and professional development to enhance skills and knowledge.
  11. Support the mission and values of Wentworth-Douglass Hospital by providing exceptional patient-centered care.
  12. Maintain a compassionate and patient-focused approach in all interactions and treatments.
  13. Act as a role model for other healthcare professionals, promoting a positive and collaborative work environment.

  15. Continuously strive to improve the virtual patient care experience and identify opportunities for process improvement.

Job Qualifications
  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team.

  • Current Licensure As A Nurse Practitioner Or Physician Assistant In The State Of New Hampshire.

  • Minimum Of 2 Years Of Clinical Experience As A Nurse Practitioner Or Physician Assistant.

  • Strong Knowledge And Understanding Of Virtual Clinical Support Technologies And Platforms.

  • Ability To Assess And Manage A Variety Of Acute And Chronic Medical Conditions Remotely, Using Evidence-Based Practice Guidelines.
