
Medication Support Coordinator

- Ensure the safe and effective use of medications for patients at Wentworth-Douglass Hospital.
- Oversee and manage all aspects of medication support for patients, including ordering, dispensing, and administering medications.
- Collaborate with healthcare team members, including physicians and nurses, to ensure proper medication management and adherence.
- Conduct patient medication reviews and provide education to patients and their families on proper medication use and potential side effects.
- Stay up to date on medication regulations and best practices to ensure compliance and safety.
- Maintain accurate and complete medication records for each patient.
- Communicate with pharmacies, insurance companies, and other healthcare facilities to coordinate medication needs for patients.
- Assist with medication inventory management, including ordering, restocking, and disposing of medications.
- Participate in interdisciplinary team meetings and provide input on medication-related issues.
- Monitor patient responses to medications and report any adverse reactions to healthcare team.
- Provide support and guidance to patients in managing their medications and following prescribed treatment plans.
- Conduct regular medication audits and make recommendations for improvements as needed.
- Train and supervise new medication support staff members.
- Maintain a high level of professionalism and compassion while interacting with patients and their families.
- Comply with all hospital policies and procedures, as well as state and federal regulations, related to medication management.
Bachelor's Degree In Healthcare Administration, Nursing, Or Related Field.
Minimum Of 2 Years Of Experience In Medication Management Or Pharmacy Coordination.
Knowledge Of State And Federal Regulations Related To Medication Management And Administration.
Strong Communication Skills And Ability To Collaborate With Healthcare Providers, Patients, And Families.
Certification As A Medication Support Coordinator (Msc) Or Willingness To Obtain Certification Within 6 Months Of Hire.
