  1. Coordinate and manage patient care activities in accordance with hospital policies and procedures.
  2. Ensure that all patient appointments, procedures, and treatments are scheduled efficiently and accurately.
  3. Communicate with patients, their families, and other healthcare professionals to provide updates and answer questions.
  4. Collaborate with other members of the healthcare team to develop and implement individualized care plans for patients.
  5. Monitor patient progress and provide support as needed to ensure their physical, emotional, and social well-being.
  6. Maintain accurate and up-to-date patient records and documentation.
  7. Assist in the coordination of hospital services, including referrals, insurance authorizations, and discharge planning.
  8. Respond to patient inquiries and concerns with empathy and professionalism.
  9. Participate in quality improvement initiatives to enhance patient care and services.
  10. Stay informed about new medical procedures, treatments, and technology to ensure the best possible care for patients.
  11. Adhere to all confidentiality and privacy regulations.
  12. Serve as a positive and professional representative of Wentworth-Douglass Hospital to patients, families, and the community.

Job Qualifications
