- Conduct and coordinate Minimum Data Set (MDS) assessments for patients in accordance with state and federal regulations.
- Ensure timely and accurate completion of all required MDS assessments for patients.
- Monitor and track patient data to identify any discrepancies or inconsistencies.
- Collaborate with interdisciplinary team members to gather and review patient information for MDS assessments.
- Communicate effectively with healthcare providers, patients, and families to obtain necessary information for MDS assessments.
- Maintain current knowledge of all state and federal regulations related to MDS assessments and ensure compliance.
- Review and analyze MDS data to identify any potential areas for improvement in patient care.
- Participate in quality improvement initiatives related to MDS assessments and patient care.
- Train and educate staff on MDS assessment processes and procedures.
- Keep accurate and organized records of all MDS assessments and related documentation.
- Work closely with billing and coding departments to ensure accurate and timely reimbursement for services provided.
- Participate in interdisciplinary team meetings to discuss patient care plans and identify any necessary changes.
- Adhere to all hospital policies and procedures and maintain patient confidentiality at all times.
- Provide guidance and support to other healthcare professionals regarding MDS assessment and documentation.
- Continuously seek opportunities for professional development and stay updated on industry changes and best practices related to MDS assessments.
Bachelor's Degree In Nursing Or Related Field.
Current And Active Rn License In The State Of New Hampshire.
Minimum Of 2 Years Of Experience As A Mds Coordinator In A Skilled Nursing Facility Or Acute Care Setting.
In-Depth Knowledge Of Medicare And Medicaid Regulations Related To The Mds Process.
