MDS Coordinator

1. Conduct and coordinate Minimum Data Set (MDS) assessments for patients in accordance with state and federal regulations.
2. Ensure timely and accurate completion of all required MDS assessments for patients.
3. Monitor and track patient data to identify any discrepancies or inconsistencies.
4. Collaborate with interdisciplinary team members to gather and review patient information for MDS assessments.
5. Communicate effectively with healthcare providers, patients, and families to obtain necessary information for MDS assessments.
6. Maintain current knowledge of all state and federal regulations related to MDS assessments and ensure compliance.
7. Review and analyze MDS data to identify any potential areas for improvement in patient care.
