Job Description

As a Family Partner, you will play a vital role in providing support, education, and resources to families of patients receiving care at our hospital. We are seeking a highly empathetic and collaborative individual who is committed to promoting patient and family-centered care.   1. Serve as a liaison between families and healthcare providers, advocating for the needs and concerns of families during their loved one's hospitalization.
  2. Provide emotional support and assistance to families during difficult and stressful times, including helping them understand medical procedures and treatments.
  3. Collaborate with hospital staff to develop and implement patient and family-centered care plans that meet the unique needs of each family.
  4. Conduct education sessions for families on hospital procedures, resources, and support services available.
  5. Connect families with community resources and support groups to help them navigate the healthcare system and cope with their loved one's illness.
  6. Maintain confidentiality and handle sensitive information with discretion.
  7. Create a welcoming and inclusive environment for families, ensuring their cultural, linguistic, and religious needs are respected.
  8. Participate in hospital committees and initiatives to promote patient and family-centered care.
  9. Stay up-to-date on healthcare policies, procedures, and regulations to effectively support families.
  10. Document interactions with families and provide regular updates to hospital staff on the needs and progress of families.
  11. Collaborate with other Family Partners and hospital staff to continuously improve the support and services provided to families.
  12. Attend training and professional development opportunities to enhance skills and knowledge in family support and advocacy.
  13. Adhere to all hospital policies and procedures, including those related to patient confidentiality and safety.
  14. Maintain a positive and professional attitude when interacting with families, hospital staff, and community partners.
  15. Foster a culture of empathy, respect, and open communication within the hospital community.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or Related Field.

  • Minimum Of 2 Years Of Experience Working With Families And Children In A Healthcare Setting.

  • Knowledge Of Community Resources And Ability To Provide Referrals To Families In Need.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Medical Staff And Families.

  • Familiarity With The Specific Needs And Challenges Faced By Families Of Patients At Newton Wellesley Hospital.
