We are currently seeking a compassionate and dedicated individual to join our team as a Community Residence Counselor for our Adult Mental Health (AMH) program.   1. Provide compassionate and dedicated care to patients with mental health disorders.
  2. Offer support and guidance to individuals in the Adult Mental Health program.
  3. Promote recovery and overall well-being of patients through the use of resources.
  4. Act as a liaison between patients and the hospital, ensuring proper communication and understanding of treatment plans.
  5. Collaborate with other members of the healthcare team to develop and implement individualized care plans.
  6. Monitor and document patient progress and behaviors to ensure appropriate treatment and support.
  7. Maintain confidentiality and adhere to all HIPAA regulations.
  8. Educate patients and their families on mental health disorders, treatment options and available resources.
  9. Participate in ongoing training and professional development opportunities to enhance knowledge and skills.
  10. Collaborate with community resources and organizations to provide comprehensive support for patients.
  11. Advocate for patients' rights and needs within the healthcare system.
  12. Adhere to hospital policies, procedures, and ethical standards in all interactions with patients and colleagues.
  13. Foster a positive and welcoming environment for patients and their families.
  14. Maintain accurate and up-to-date patient records and documentation.
  15. Adhere to all safety protocols and maintain a safe and clean environment for patients.

Job Qualifications
  • At Least 2 Years Of Experience Working In A Mental Health Or Residential Setting.

  • Minimum Of A Bachelor's Degree In Psychology, Social Work, Or A Related Field.

  • Knowledge Of And Experience With Mental Health Diagnoses, Treatment Plans, And Crisis Management.

  • Strong Communication And Interpersonal Skills, With The Ability To Work Effectively With A Diverse Population.

  • Familiarity With Electronic Medical Records And Documentation Procedures.
