- Dispense medication and provide medication counseling to patients in an ambulatory setting.
- Collaborate with healthcare providers to optimize medication therapy and improve patient outcomes.
- Perform medication therapy management and assess medication regimens for appropriateness and safety.
- Monitor patient medication adherence and provide interventions as needed.
- Educate patients and healthcare providers on medication therapy, drug interactions, and potential side effects.
- Maintain accurate and up-to-date patient records and documentation.
- Comply with all state and federal laws and regulations regarding medication dispensing and patient privacy.
- Stay current on new medications, drug therapies, and healthcare trends through continuing education.
- Maintain inventory and order necessary medications and supplies for the pharmacy.
- Participate in quality improvement initiatives and assist in developing and implementing protocols and guidelines for medication use.
- Provide guidance and support to pharmacy technicians and other staff members.
- Collaborate with insurance companies and other third-party payers to ensure proper coverage for patients.
- Provide excellent customer service to patients and healthcare providers.
- Participate in on-call and after-hours duties as needed.
- Adhere to all hospital policies and procedures and maintain a safe and clean work environment.
- Attend departmental and hospital meetings and trainings as required.
- Maintain a professional and compassionate attitude towards patients and their families.
- Uphold the hospital's mission, vision, and values in all job duties.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution
Current State Licensure As A Pharmacist
Minimum Of 2 Years Of Experience In An Ambulatory Or Outpatient Pharmacy Setting
Strong Knowledge Of Medication Therapy Management And Ability To Provide Patient Education And Counseling
Experience With Electronic Health Records And Proficiency In Computer Skills For Documentation And Medication Reconciliation
