- Coordinate and manage the care of patients at Wentworth-Douglass Hospital.
- Advocate for the physical, emotional, and social needs of patients.
- Collaborate with a multidisciplinary team to develop and implement care plans for patients.
- Assess and reassess patients' needs and adjust care plans accordingly.
- Communicate effectively with patients, families, and healthcare professionals.
- Ensure timely and accurate documentation of patient care and progress.
- Provide emotional support and counseling to patients and their families.
- Identify and address any barriers to care, such as financial or social issues.
- Collaborate with community resources to provide additional support and services for patients.
- Stay up-to-date with industry regulations and best practices in case management.
- Participate in meetings and trainings related to case management.
- Maintain confidentiality and adhere to ethical standards in all interactions.
- Represent Wentworth-Douglass Hospital in a professional and compassionate manner.
- Continuously evaluate and improve patient care processes.
- Provide guidance and support to other members of the case management team.
Bachelor's Degree In Social Work, Psychology, Nursing, Or Related Field.
Minimum Of 2 Years Experience In Case Management, Preferably In A Healthcare Setting.
Knowledge Of Medicare And Medicaid Regulations And Experience With Insurance Authorization Processes.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams.
Certification In Case Management (Ccm) Preferred.
