- Provide compassionate and high-quality care to patients and their families in the hospice program.
- Serve as a liaison between patients, families, and healthcare providers to coordinate and facilitate care.
- Conduct initial assessments and ongoing evaluations of patients' physical, emotional, and spiritual needs.
- Develop and implement individualized care plans based on patients' needs and goals.
- Educate patients and families on the hospice program and services available.
- Collaborate with interdisciplinary team members to ensure comprehensive and holistic care for patients.
- Monitor patients' condition and provide ongoing support and comfort measures.
- Communicate effectively with patients, families, and healthcare providers to advocate for the best possible quality of life for patients.
- Maintain accurate and up-to-date documentation of patient care and progress.

- Act as a resource and mentor for other healthcare professionals, providing guidance and support as needed.

Bachelor's Degree In Nursing: A Uchicago Medicine Home Health Nurse Liaison Must Possess A Bachelor's Degree In Nursing From An Accredited Institution. This Degree Provides A Strong Foundation In Nursing Theory And Practice, As Well As A Comprehensive Understanding Of Patient Care In The Home Health Setting.
Valid Nursing License: A Candidate For This Position Must Hold A Valid Nursing License From The State In Which They Will Be Working. This Ensures That They Have Met All Necessary Requirements To Practice Nursing And Are Knowledgeable About Current Regulations And Standards.
Previous Home Health Experience: To Be Successful In This Role, The Candidate Should Have Previous Experience Working In A Home Health Setting. This Will Allow Them To Effectively Assess Patient Needs, Coordinate Care, And Provide Education To Patients And Their Families.
Hospice Certification: The Ideal Candidate Should Also Possess A Certification In Hospice Care. This Demonstrates A Specialized Knowledge And Skill Set In Caring For Patients With Life-Limiting Illnesses And Providing Comfort And Support To Their Families.
Excellent Communication Skills: A Uchicago Medicine Home Health Nurse Liaison Must Have Exceptional Communication Skills To Effectively Collaborate With Patients, Families, And Interdisciplinary Teams. They Must Also Be Able To Document Patient Care Accurately And Communicate Effectively With Physicians And Other Healthcare Providers.
