
(RN) Registered Nurse - HC Inpatient Transition

- Provide compassionate and skilled nursing care to patients transitioning from inpatient to home care.
- Collaborate with interdisciplinary teams to assess patient needs and develop individualized care plans.
- Perform comprehensive patient assessments, including physical, psychological, and social factors, to ensure safe and effective care.
- Administer medications, treatments, and procedures in accordance with established protocols and standards of care.
- Educate patients and their families on disease management, treatment plans, and self-care techniques.
- Coordinate with home health agencies and other community resources to ensure a smooth transition for patients.
- Monitor and document patient progress and report any changes or concerns to the healthcare team.
- Adhere to all infection control and safety protocols to maintain a safe and healthy environment for patients and staff.
- Participate in quality improvement initiatives and contribute to the development of evidence-based practices.
- Maintain accurate and up-to-date patient records and documentation.
- Demonstrate empathy, patience, and understanding towards patients and their families during a vulnerable time.
- Stay informed on current nursing practices, procedures, and standards of care.
- Uphold the values and mission of the University of California, San Diego, and the Home Care department.
- Maintain confidentiality and adhere to ethical standards in all interactions with patients, families, and colleagues.
- Act as an advocate for patients and their families, ensuring their needs and preferences are considered in their care.
Current Rn License In The State Of California.
Minimum Of 2 Years Of Experience Working As An Rn In An Acute Care Hospital Setting.
Bachelor's Degree In Nursing From An Accredited Institution.
Experience In Patient Care Coordination And Discharge Planning.
Strong Communication Skills And Ability To Work Collaboratively With Interdisciplinary Teams.
