  1. Facilitate and coordinate kidney transplants for patients, ensuring all necessary steps and protocols are followed.

  3. Collaborate with the multidisciplinary team to develop and implement individualized care plans for patients.
  4. Maintain accurate and thorough documentation of patient information, procedures, and outcomes.
  5. Educate patients and their families on the transplant process, including pre- and post-operative care and potential complications.
  6. Serve as a liaison between patients, families, and healthcare professionals, providing clear communication and addressing any concerns or questions.
  7. Monitor and assess patients' physical and emotional well-being, providing appropriate support and interventions as needed.
  8. Stay current on advancements and changes in the field of transplantation, implementing evidence-based practices in patient care.
  9. Adhere to all regulatory and accreditation standards, ensuring compliance with policies and procedures.
  10. Demonstrate professionalism, empathy, and compassion in all interactions with patients, families, and colleagues.
  11. Maintain confidentiality of patient information and uphold ethical standards in all aspects of the job.
  12. Participate in quality improvement initiatives and contribute to the development and implementation of best practices for kidney transplant care.
  13. Serve as a mentor and resource for other members of the healthcare team, including nurses and support staff.
  14. Adhere to a schedule that may include on-call responsibilities and occasional weekends or holidays.
  15. Continuously seek opportunities for professional development and growth, staying up-to-date on industry trends and advancements.

Job Qualifications
  • Bachelor's Degree In Nursing From An Accredited Institution.

  • Current And Valid Registered Nurse (Rn) License In The State Of California.

  • Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In Nephrology And/Or Transplant.

  • Advanced Certification In Nephrology Or Transplant Nursing (E.g. Cnn Or Cctn).

  • Strong Communication And Organizational Skills, With The Ability To Collaborate With Multi-Disciplinary Teams And Provide Patient Education And Support.
