  14. Assist with data collection and reporting to track patient outcomes and program effectiveness.

Job Qualifications
  • Bachelor's Degree In A Related Field (Such As Healthcare Administration, Social Work, Or Public Health)

  • Minimum Of 3-5 Years Of Experience In A Patient Navigation Or Care Coordination Role

  • Knowledge Of Healthcare Systems And Resources, Particularly Within The University Of California Healthcare Network

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Engage And Support Patients From Diverse Backgrounds

  • Demonstrated Ability To Work Independently And Collaboratively, With A Focus On Problem-Solving And Improving Patient Outcomes
