  1. Conduct thorough assessments of elderly patients to identify their individual needs and develop personalized treatment plans.
  2. Provide individual and group therapy to patients using evidence-based practices.
  3. Utilize a holistic approach to address the physical, emotional, and social needs of patients.
  4. Collaborate with interdisciplinary team members to ensure coordinated and comprehensive care for patients.
  5. Maintain accurate and up-to-date patient records and documentation.
  6. Monitor and evaluate patient progress, adjusting treatment plans as needed.
  7. Educate and support patients and their families on coping strategies and available resources.
  8. Advocate for the needs and rights of elderly patients, including addressing any barriers to care.
  9. Stay current on advancements and trends in geriatric mental health to provide the best possible care to patients.
  10. Adhere to ethical and legal standards in all aspects of practice.
  11. Attend regular team meetings and participate in case conferences to discuss patient progress and treatment plans.
  12. Serve as a resource and mentor to other team members, including interns and students.
  13. Collaborate with community organizations and agencies to coordinate care and resources for patients.
  14. Maintain confidentiality and professionalism in all interactions with patients and their families.
  15. Continuously seek opportunities for professional development and growth.

Job Qualifications
  • Master's Degree In Social Work From An Accredited University, With A Focus On Clinical Practice And Specialization In Mental Health.

  • Minimum Of 5 Years Of Experience As A Licensed Clinical Social Worker, Preferably In A Hospital Or Healthcare Setting.

  • Current And Active Licensure As A Clinical Social Worker In The State Of California.

  • Extensive Knowledge And Understanding Of Behavioral Health Issues, Including Diagnosis And Treatment Of Mental Health Disorders In Older Adults.

  • Strong Communication, Collaboration, And Advocacy Skills To Work Effectively With A Multidisciplinary Team And Provide Support To Patients And Their Families.
