  1. Oversee and manage a team of home health care professionals, including nurses, therapists, and aides, to ensure the delivery of high-quality and compassionate care to patients.
  2. Develop and implement policies, procedures, and standards for the home health care team in accordance with the organization's mission and values.
  3. Monitor and evaluate the performance of team members, providing feedback and coaching as needed to ensure a high level of quality and efficiency.
  4. Collaborate with other healthcare providers, such as physicians, social workers, and case managers, to coordinate and optimize patient care.
  5. Conduct regular meetings and trainings with the home health care team to promote continuous education and professional development.
  6. Maintain accurate and up-to-date patient records and documentation, ensuring compliance with all relevant regulations and standards.
  7. Serve as a liaison between patients, caregivers, and other healthcare professionals to ensure effective communication and coordination of care.
  8. Participate in the recruitment, hiring, and onboarding process for new team members, ensuring a diverse and qualified workforce.
  9. Monitor and manage the budget for the home health care team, including staffing, supplies, and equipment.
  10. Stay current on industry trends and best practices in home health care, continuously seeking opportunities to improve and enhance the services provided by the team.
  11. Foster a positive and supportive work environment that promotes teamwork, collaboration, and a commitment to the organization's mission and values.
  12. Adhere to all applicable laws, regulations, and ethical standards while carrying out job responsibilities.

Job Qualifications
  • Current And Active Registered Nurse (Rn) License In The State Of Employment.

  • Bachelor's Degree In Nursing (Bsn) Or Higher From An Accredited Institution.

  • Minimum Of 3-5 Years Of Experience In Home Health Or Related Clinical Setting.

  • Proven Leadership Skills And Experience Managing A Team Of Clinical Professionals.

  • Excellent Communication, Organizational, And Problem-Solving Skills.
