  4. Collaborate with other healthcare professionals to coordinate and manage patient care.
  5. Conduct regular home visits to assess patient progress and update care plans as needed.

  7. Keep accurate and thorough documentation of patient care and progress.
  8. Educate patients and families on their conditions and how to manage them at home.
  9. Serve as a resource and support for patients and families, addressing any concerns or questions.
  10. Participate in team meetings and case conferences to discuss patient progress and develop care strategies.
  11. Continuously strive to improve and enhance patient outcomes and satisfaction.
  12. Stay current with advancements and changes in the healthcare field, implementing best practices into patient care.
  13. Maintain a positive and professional attitude, promoting a supportive and compassionate environment for patients and team members.
  14. Adhere to company policies and procedures, maintaining confidentiality and ethical standards.

Job Qualifications
  • Bachelor's Degree In Nursing (Bsn) Or Higher From An Accredited Institution.

  • Current And Unrestricted Registered Nurse (Rn) License In The State Of Employment.

  • Minimum Of 3 Years Clinical Experience As A Registered Nurse.

  • Prior Experience In Care Coordination, Case Management, Or Utilization Management.

  • Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team And Effectively Communicate With Patients And Healthcare Providers.
