  1. Provide compassionate and high-quality care to patients in their homes using clinical expertise and case management skills.
  2. Conduct thorough assessments of patient needs and develop individualized care plans.
  3. Collaborate with interdisciplinary team members to ensure the continuity of care and patient well-being.
  4. Monitor and evaluate patient progress, making necessary adjustments to care plans as needed.
  5. Educate patients and their families on disease management, treatment plans, and available resources.
  6. Maintain accurate and timely documentation of patient interactions and interventions.
  7. Ensure compliance with all regulatory and company policies and procedures.
  8. Communicate effectively with patients, caregivers, and healthcare providers to promote positive outcomes.

  10. Serve as an advocate for patients, ensuring they receive the best possible care and support.
  11. Collaborate with community resources to coordinate services and support for patients.

Job Qualifications
  • Current Rn License: The Ideal Candidate For This Position Must Hold A Valid And Current Rn License In The State In Which They Will Be Practicing.

  • Minimum Of 3 Years Of Clinical Experience: The Candidate Should Have At Least 3 Years Of Experience Working As A Registered Nurse In A Clinical Setting, Preferably In A Field Case Management Role.

  • Case Management Certification: A Certification In Case Management, Such As Ccm Or Cdms, Is Preferred And Demonstrates The Candidate's Knowledge And Skills In This Specialized Field.

  • Strong Communication And Interpersonal Skills: As A Field Case Manager, The Candidate Will Be Working With A Variety Of Individuals, Including Patients, Healthcare Providers, And Insurance Representatives. Excellent Communication And Interpersonal Skills Are Essential For Success In This Role.

  • Knowledge Of Healthcare Systems And Regulations: A Thorough Understanding Of Healthcare Systems And Regulations, Including Insurance Coverage And Reimbursement, Is Crucial For A Field Case Manager To Effectively Navigate And Advocate For Patients Within The Healthcare System.
