  4. Maintain patient records: The Patient Care Coordinator will be responsible for keeping accurate and up-to-date patient records, including medical history, treatment plans, and insurance information.
  5. Collaborate with healthcare providers: The Patient Care Coordinator will work closely with healthcare providers to ensure that patients receive the necessary care and treatment.
  6. Advocate for patients: The Patient Care Coordinator will serve as an advocate for patients, ensuring that their needs are met and their rights are protected.
  7. Monitor patient progress: The Patient Care Coordinator will regularly monitor patient progress and make adjustments to care plans as needed.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Nursing, Or A Related Field.

  • Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In A Patient Care Coordination Role.

  • Strong Knowledge Of Healthcare Regulations, Such As Hipaa And Medicare/Medicaid.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work With A Diverse Group Of Patients And Healthcare Professionals.

  • Proficiency In Electronic Health Records (Ehr) And Other Healthcare Software Systems.
