Nurse Practitioner House Calls

Our ideal candidate is an experienced professional with a strong dedication to providing excellent healthcare in the comfort of our patients’ homes.The Nurse Practitioner will be part of a collaborative team of medical and non-medical providers, dedicated to providing comprehensive, high quality care to our patients. The Nurse Practitioner will be responsible for conducting comprehensive care visits to our homebound patients, providing patient education, and writing clear and comprehensive care plans.The successful candidate will possess a valid RN license and a valid Nurse Practitioner license in the state of practice. In addition, the ideal candidate will have experience in providing home-based care, at least 2 years of experience working with geriatric and/or chronically ill patients, and excellent communication skills.
