We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in providing high-quality, in-home healthcare services to patients in Multnomah County.   1. Provide high-quality, in-home healthcare services to patients in Multnomah County.
  2. Conduct comprehensive assessments of patients' medical conditions and develop personalized care plans.

Job Qualifications
  • Graduate Degree In Nursing Or Physician Assistant Studies From An Accredited Program

  • Current Certification And Licensure As A Nurse Practitioner Or Physician Assistant In The State Of Multnomah County

  • Minimum Of 2 Years Of Clinical Experience In Primary Care, Urgent Care, Or Related Setting

  • Strong Clinical Knowledge And Skills, Including Ability To Diagnose And Treat A Wide Range Of Acute And Chronic Conditions

  • Excellent Communication And Interpersonal Skills, With The Ability To Build Rapport And Establish Trust With Patients And Colleagues.
