- Conduct in-home visits to provide comprehensive and personalized care to patients.
- Perform physical exams, diagnose and treat acute and chronic conditions, and prescribe appropriate medications.
- Develop and implement personalized care plans for patients, in collaboration with the primary care physician.
- Communicate with patients and their families to educate them on their health conditions and treatment plans.
- Maintain accurate and up-to-date medical records and documentation.

Graduate Degree In Nurse Practitioner Or Physician Assistant Studies From An Accredited Program.
Current And Active Np Or Pa Licensure In The State Of Practice.
Minimum Of 3 Years Of Clinical Experience, Preferably In A Primary Care Or Geriatric Setting.
Strong Communication And Interpersonal Skills To Effectively Engage With Patients And Other Healthcare Professionals.
Ability To Work Independently And Manage A Caseload, While Also Collaborating With A Multidisciplinary Team.
