  1. Coordinate and manage care for inpatient population, ensuring delivery of high-quality, cost-effective care.
  2. Serve as a liaison between patients, families, and healthcare providers.
  3. Utilize strong clinical background to assess and address patient needs.
  4. Communicate effectively with patients, families, and healthcare providers to ensure understanding and collaboration.
  5. Monitor and evaluate patient progress and make necessary adjustments to care plan.
  6. Collaborate with interdisciplinary team to develop and implement individualized care plans.
  7. Educate patients and families on treatment plans and medication management.
  8. Ensure compliance with all relevant regulations, policies, and procedures.
  9. Document all patient interactions and maintain accurate records.
  10. Participate in quality improvement initiatives to enhance patient outcomes.
  11. Provide support and guidance to patients and families during transitions of care.
  12. Continuously assess and identify opportunities for improvement in care delivery.
  13. Stay updated on current healthcare trends, policies, and procedures.
  14. Foster a positive and compassionate environment for patients and team members.
  15. Adhere to ethical and professional standards of practice.

Job Qualifications
  • Strong Clinical Assessment And Critical Thinking Skills
