  2. Serve as a liaison between patients and healthcare providers, ensuring effective communication and coordination of care.
  3. Conduct comprehensive assessments of patients' health needs and create individualized care plans.
  4. Monitor and evaluate patient progress, making recommendations for adjustments to treatment plans as needed.
  5. Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for patients.
  6. Educate patients and their families about their health conditions, treatment options, and self-care strategies.
  7. Maintain accurate and up-to-date patient records and documentation.
  8. Adhere to all regulatory and compliance guidelines and standards, including HIPAA.
  9. Participate in ongoing professional development and training opportunities to enhance clinical skills and knowledge.

Job Qualifications
  • Bachelor's Degree In Nursing: A Field Hss Clinical Coordinator Rn Should Have A Minimum Of A Bachelor's Degree In Nursing From An Accredited University Or College.

  • Active Rn License: The Candidate Should Have An Active And Unrestricted Registered Nurse (Rn) License In The State Where They Will Be Working.

  • Minimum Of 3 Years Of Clinical Experience: A Field Hss Clinical Coordinator Rn Should Have A Minimum Of 3 Years Of Experience Working As A Registered Nurse In A Clinical Setting.

  • Strong Communication And Leadership Skills: The Ideal Candidate Should Possess Excellent Communication And Leadership Skills To Effectively Coordinate And Collaborate With Healthcare Providers, Patients, And Other Stakeholders.

  • Knowledge Of Healthcare Regulations And Compliance: The Candidate Should Have A Thorough Understanding Of Healthcare Regulations, Including Hipaa And Other Compliance Requirements, To Ensure That All Clinical Activities Are Performed In Accordance With These Standards.
