- Conduct community outreach and establish relationships with community members to identify health needs and concerns.
- Provide support and assistance to individuals and families in navigating the healthcare system.
- Educate community members on health promotion and disease prevention strategies.
- Collaborate with healthcare providers and community organizations to develop and implement health programs.
- Advocate for individuals and families to ensure they have access to necessary healthcare services.
- Conduct home visits and assist in connecting individuals with resources and services.
- Document and maintain accurate records of community interactions and progress.
- Attend trainings and meetings to stay updated on health policies and procedures.
- Serve as a liaison between community members and healthcare providers.
- Promote and encourage participation in health screenings and events.
- Monitor and track health outcomes for individuals and the community as a whole.
- Maintain confidentiality and adhere to ethical standards in all interactions.
- Continuously evaluate and improve community health programs and interventions.
- Collaborate with other members of the healthcare team to provide holistic care for community members.
- Demonstrate empathy, compassion, and cultural sensitivity in all interactions with community members.
High School Diploma Or Equivalent: A Minimum Of A High School Diploma Or Equivalent Is Required For A Field Community Health Worker Position At Unitedhealth Group. This Demonstrates Basic Education And Communication Skills Necessary For The Job.
Experience In Community Health Or Public Health: Previous Experience In Community Health Or Public Health Is Preferred For This Role. Candidates With Experience In Health Promotion, Community Outreach, Or Working With Underserved Populations Will Be Highly Valued.
Knowledge Of Healthcare Systems And Resources: A Strong Understanding Of Healthcare Systems And Available Resources Is Essential For A Field Community Health Worker. This Includes Knowledge Of Insurance Coverage, Government Programs, And Community-Based Organizations.
Excellent Communication Skills: Strong Communication Skills, Both Written And Verbal, Are Crucial For This Role. Field Community Health Workers Must Be Able To Effectively Communicate With Diverse Populations And Collaborate With Various Healthcare Professionals.
Bilingual Proficiency: Fluency In A Second Language, Particularly Spanish, Is Highly Desired For This Position. This Allows For Better Communication And Outreach To Diverse Communities, Which Is A Key Aspect Of The Role.
