  1. Coordinate and facilitate patient care in a variety of home settings, ensuring high-quality and personalized care for each patient.
  2. Serve as a liaison between patients, caregivers, and healthcare providers to ensure effective communication and coordination of care.
  3. Conduct comprehensive assessments of patients' needs, including physical, emotional, and social factors, to develop individualized care plans.
  4. Collaborate with interdisciplinary team members, including physicians, nurses, therapists, and social workers, to coordinate and implement patient care plans.
  5. Provide education and support to patients and their families to promote understanding of their medical conditions and treatment plans.
  6. Monitor and evaluate patients' progress and adjust care plans as needed to ensure optimal outcomes.
  7. Maintain accurate and up-to-date documentation of all patient interactions and care plans.
  8. Work closely with community resources to facilitate access to additional support services for patients.
  9. Adhere to all relevant regulations, policies, and procedures to ensure compliance and maintain patient confidentiality.
  10. Continuously seek opportunities for process improvement and collaborate with team members to enhance patient care delivery.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field: A Minimum Of A Bachelor's Degree In Nursing Or A Related Field Is Typically Required For A Field Based Patient Care Coordinator Position. This Demonstrates A Strong Foundation In Healthcare And Clinical Knowledge.

  • Active Nursing License: In Addition To A Degree, A Valid And Active Nursing License Is Often Required For This Role. This Ensures That The Candidate Has Met All Necessary Requirements To Practice Nursing And Can Provide Direct Patient Care.

  • Experience In Healthcare Or Patient Coordination: Previous Experience In A Healthcare Setting, Such As Working As A Nurse Or Patient Coordinator, Is Highly Valued For This Role. It Demonstrates The Candidate's Understanding Of The Healthcare Industry And Their Ability To Manage Patient Care.

  • Strong Communication And Interpersonal Skills: As A Field Based Position, The Patient Care Coordinator Will Need To Communicate Effectively With Patients, Healthcare Providers, And Other Team Members. Strong Communication And Interpersonal Skills Are Essential For Building Relationships And Ensuring Quality Patient Care.
