1. Coordinate and manage healthcare services for UnitedHealth Group members in the field.
2. Utilize clinical expertise to improve patient outcomes and ensure high-quality healthcare services.
3. Conduct field visits to assess member needs and develop personalized care plans.
4. Collaborate with healthcare providers to ensure seamless coordination of care for members.
5. Monitor and track member progress, identify any issues or concerns, and implement appropriate interventions.
6. Serve as a point of contact for members, families, and healthcare providers to address any questions or concerns.
7. Educate members and their families on available healthcare resources and services.
8. Maintain accurate and confidential records of member interactions and progress.
9. Stay up-to-date with industry developments and best practices in healthcare coordination.
10. Communicate regularly with team members and participate in team meetings to discuss member care and identify areas for improvement.
