
Field Based Community Health Worker

- Build and maintain relationships with community members, including regular outreach and follow-up to ensure their needs are being met.
- Conduct comprehensive assessments of individuals' social and health needs and develop personalized care plans to address them.
- Educate community members on resources available to them and assist with accessing those resources.
- Collaborate with healthcare providers and community organizations to coordinate care and services for community members.
- Facilitate support groups and workshops to promote health education and self-management skills.
- Maintain accurate and up-to-date documentation of all interactions and interventions with community members.
- Monitor and track progress of community members' health goals and provide ongoing support and guidance.
- Identify and address social determinants of health that may be impacting community members' well-being.
- Advocate for community members and connect them with appropriate resources to address social and health disparities.
- Stay informed about current community health initiatives and trends, and make recommendations for program improvements.
- Collaborate with team members and attend regular meetings to discuss community needs and develop strategies for addressing them.
- Adhere to all ethical and confidentiality guidelines when working with community members.
Bachelor's Degree In A Related Field: A Strong Foundation In Public Health, Social Work, Or A Related Field Is Essential For This Position. A Bachelor's Degree Demonstrates The Necessary Knowledge And Skills To Effectively Serve As A Community Health Worker.
Previous Experience In Community Outreach: Candidates Should Have Experience Working In The Community, Whether Through Volunteer Work Or Previous Employment. This Experience Will Help Them Understand The Unique Needs And Challenges Of The Populations They Will Be Serving.
Strong Communication And Interpersonal Skills: As A Community Health Worker, You Will Be Interacting With A Diverse Group Of Individuals And Organizations. Therefore, Excellent Communication And Interpersonal Skills Are Essential For Building Relationships And Effectively Delivering Health Education And Resources.
Bilingual Or Multilingual Proficiency: Many Communities Served By Unitedhealth Group Have Diverse Populations With A Variety Of Languages Spoken. Being Fluent In Multiple Languages Can Help You Better Connect And Serve These Communities.
Knowledge Of Local Resources And Services: Community Health Workers Need To Be Familiar With The Resources And Services Available In The Areas They Serve. This Includes Knowledge Of Local Clinics, Hospitals, Social Service Agencies, And Other Community-Based Organizations That Can Help Address The Health Needs Of The Population.
