Field Based Community Health Worker

3. Educate community members on resources available to them and assist with accessing those resources.
4. Collaborate with healthcare providers and community organizations to coordinate care and services for community members.
5. Facilitate support groups and workshops to promote health education and self-management skills.
6. Maintain accurate and up-to-date documentation of all interactions and interventions with community members.
7. Monitor and track progress of community members' health goals and provide ongoing support and guidance.
8. Identify and address social determinants of health that may be impacting community members' well-being.
9. Advocate for community members and connect them with appropriate resources to address social and health disparities.
10. Stay informed about current community health initiatives and trends, and make recommendations for program improvements.
11. Collaborate with team members and attend regular meetings to discuss community needs and develop strategies for addressing them.
12. Adhere to all ethical and confidentiality guidelines when working with community members.
