- Conduct home visits and community outreach to connect with individuals and families in need of support.
- Educate and empower individuals and families to make positive lifestyle changes that promote better health and prevent chronic diseases.
- Collaborate with healthcare providers to develop personalized care plans for each individual, taking into account their unique needs and social determinants of health.
- Advocate for community resources and services that can support the health and well-being of our members.
- Build and maintain trusting relationships with individuals and families from diverse backgrounds, cultures, and socioeconomic statuses.
- Utilize effective communication skills to educate and motivate individuals to take charge of their health and make informed healthcare decisions.
- Organize and facilitate group workshops and health education sessions in community settings.
- Document and track progress of individuals and families, ensuring all necessary data is accurately recorded and reported.
- Continuously stay informed about local resources, policies, and initiatives related to community health.
- Participate in team meetings and trainings to enhance knowledge and skills related to community health work.
- Collaborate with other community health workers and healthcare professionals to develop and implement strategies for promoting health equity and addressing health disparities.
- Identify and report any barriers or challenges that may impact the health and well-being of our members.
- Adhere to all ethical and confidentiality standards in working with individuals and families.
- Serve as a positive role model and ambassador for UnitedHealth Group, promoting our mission and values in the community.

High School Diploma Or Equivalent: A Minimum Education Requirement For A Field Based Community Health Worker Job At Unitedhealth Group Is A High School Diploma Or Equivalent. This Demonstrates A Basic Level Of Academic Achievement And Knowledge.
Certification In Community Health Work: Unitedhealth Group May Require Candidates To Hold A Certification In Community Health Work From An Accredited Program. This Certification Shows That The Individual Has Received Specialized Training In The Field And Is Knowledgeable About Community Health Principles And Practices.
Experience In Community Outreach Or Health Education: Candidates With Experience In Community Outreach Or Health Education Will Be Preferred For A Field Based Community Health Worker Job At Unitedhealth Group.
