  1. Provide comprehensive diabetes education and support to individuals living with diabetes.
  2. Conduct thorough assessments of patients' diabetes management needs and create individualized education plans.
  3. Educate patients on diabetes self-management techniques, including medication management, blood glucose monitoring, and lifestyle modifications.
  4. Collaborate with other healthcare professionals to ensure coordinated and effective care for patients.
  5. Utilize evidence-based practices and the latest research to inform diabetes education and support strategies.
  6. Monitor and track patient progress and outcomes, making necessary adjustments to education plans as needed.
  7. Serve as a resource and expert on diabetes management for patients, families, and other healthcare professionals.
  8. Stay current on developments and advancements in diabetes education and incorporate new techniques and technologies into practice.
  9. Maintain accurate and up-to-date documentation of patient education and progress.
  10. Act as an advocate for patients, promoting self-management and empowering them to take control of their diabetes.
  11. Participate in quality improvement initiatives to enhance diabetes education and support services.
  12. Adhere to all regulatory and compliance standards in the delivery of diabetes education.
  13. Attend trainings, conferences, and workshops to continue professional development and maintain CDCES certification.
  14. Collaborate with community organizations and resources to provide additional support and education for patients with diabetes.
  15. Promote the mission and values of UnitedHealth Group and contribute to a positive and collaborative team environment.

Job Qualifications
  • Current Rn Licensure In The State Of Employment

  • Bachelor's Degree In Nursing From An Accredited School

  • Certified Diabetes Educator (Cde) Credential From The National Certification Board For Diabetes Educators (Ncbde)

  • Minimum Of 3 Years Of Clinical Experience In Diabetes Management

  • Strong Knowledge Of Diabetes Management Principles, Including Medication Management, Lifestyle Modifications, And Patient Education Methods
