As a Nurse Navigator, you will play a critical role in guiding patients through their urological journey, providing education and support, and collaborating with a multidisciplinary team to ensure the best possible outcomes.   1. Provide expert guidance and support to patients with urological conditions throughout their healthcare journey.
  2. Collaborate with urologists, primary care physicians, and other healthcare professionals to create and implement individualized treatment plans for patients.
  3. Facilitate communication between patients and their healthcare team to ensure all parties are informed and involved in the treatment process.
  4. Educate patients and their families about urological conditions, treatment options, and potential side effects.
  5. Assist in coordinating appointments, tests, and procedures for patients to ensure timely and efficient care.
  6. Monitor patient progress and provide ongoing support and resources to address any concerns or complications.
  7. Serve as an advocate for patients to ensure their needs and preferences are prioritized and addressed.
  8. Maintain accurate and thorough documentation of patient interactions and treatment plans.
  9. Participate in multidisciplinary team meetings to discuss patient cases and contribute valuable insights and recommendations.
  10. Stay up-to-date on the latest advancements and best practices in urological care to provide the highest quality of care to patients.
  11. Foster a compassionate and supportive environment for patients and their families during what can be a challenging and stressful time.
  12. Collaborate with community resources to provide patients with additional support and resources as needed.
  13. Adhere to all regulatory and compliance guidelines to ensure the safety and well-being of patients.
  14. Continuously evaluate and improve the Nurse Navigator program to enhance patient satisfaction and outcomes.

Job Qualifications
  • Bachelor Of Science In Nursing (Bsn) Degree From An Accredited Institution.

  • Current Registered Nurse (Rn) License In The State Of Employment.

  • Minimum Of 2-3 Years Of Experience In Urology Nursing, With A Strong Understanding Of Urological Conditions And Treatments.

  • Certified Urology Nurse (Curn) Certification Preferred.

  • Excellent Communication And Patient Advocacy Skills, With The Ability To Provide Compassionate And Comprehensive Care To Patients And Their Families.
