We are looking for a team player who is an excellent communicator, organized, and has the ability to work in a fast-paced environment. The successful candidate will be able to offer supportive services to ensure patient access to care and support, along with providing effective case management services.To be successful in this role, you should have a Bachelor’s Degree in Social Work or a related field, as well as at least two years of experience in case management, social services, or healthcare. You should be knowledgeable about healthcare regulations and have a working knowledge of the medical terminology. A valid license in the state of practice is also required.In this role, you will be responsible for the evaluation, assessment, and case management of the health care needs of our patients. You will work closely with the patients to coordinate services, monitor progress, and ensure compliance with the regulations.
