  1. Develop and implement personalized care plans for patients in accordance with their healthcare needs and goals.
  2. Serve as an advocate for patients, ensuring their needs and concerns are addressed and communicated effectively to the healthcare team.
  3. Coordinate and collaborate with healthcare providers, including physicians, specialists, and other members of the interdisciplinary team, to ensure continuity of care.
  4. Conduct regular assessments of patient progress and make necessary adjustments to care plans as needed.
  5. Monitor and manage patient outcomes, including tracking and reporting on key metrics and quality measures.
  6. Educate patients and their families on their healthcare options, resources, and treatment plans.
  7. Communicate effectively with patients, families, and healthcare team members to ensure a positive and collaborative care experience.
  8. Stay up-to-date on industry standards and best practices in case management and nursing to provide high-quality care.
  9. Maintain accurate and detailed patient records and documentation in compliance with legal and regulatory requirements.
  10. Participate in case conferences and team meetings to discuss and review patient care plans and progress.
  11. Collaborate with insurance companies and other third-party payers to ensure proper coverage and reimbursement for patient care.
  12. Serve as a resource and mentor to other healthcare team members, including nurses, social workers, and support staff.
  13. Adhere to ethical and professional standards, including maintaining patient confidentiality and respecting patient rights.
  14. Continuously evaluate and improve case management processes and procedures to enhance efficiency and quality of care.
  15. Contribute to a positive and collaborative work environment by fostering open communication and teamwork among colleagues.

Job Qualifications
  • Valid Rn License: A Valid Registered Nurse License Is A Must-Have Qualification For Any Rn Case Manager Position At Tenet Healthcare. This Ensures That The Individual Has The Necessary Education, Training, And Skills To Provide Quality Patient Care.

  • Bachelor's Degree In Nursing: Tenet Healthcare Typically Requires A Bachelor's Degree In Nursing For Their Rn Case Manager Positions. This Provides A Strong Foundation In Nursing Theory And Practice, As Well As Leadership And Management Skills Necessary For The Role.

  • Case Management Certification: Tenet Healthcare May Also Prefer Candidates Who Hold A Certification In Case Management, Such As The Certified Case Manager (Ccm) Or Accredited Case Manager (Acm) Credentials. This Demonstrates A Specialized Knowledge And Expertise In The Field Of Case Management.

  • Previous Nursing Experience: Prior Experience As A Registered Nurse Is A Key Qualification For An Rn Case Manager Position At Tenet Healthcare. This Experience Allows The Individual To Have A Strong Understanding Of Patient Care, Treatment Plans, And Medical Procedures.

  • Strong Communication And Organizational Skills: As A Case Manager, The Individual Will Be Responsible For Coordinating Care And Communicating With Patients, Families, And Healthcare Providers. Therefore, Strong Communication And Organizational Skills Are Essential For Success In This Role.
