In this role, you will be responsible for overseeing the case management program across multiple facilities, ensuring optimal patient outcomes and financial performance. Develop and implement strategic plans for the case management program across multiple facilities.
Ensure that all case managers are providing high-quality and efficient services to patients.
Monitor and analyze data to identify areas for improvement and implement necessary changes to optimize patient outcomes and financial performance.
Collaborate with cross-functional teams, including physicians, nurses, and administrators, to ensure seamless coordination of care and effective communication.
Oversee the hiring, training, and development of case management staff.
Develop and maintain relationships with external stakeholders, such as insurance companies and community organizations, to support the case management program.
Stay up-to-date on industry trends, regulations, and best practices related to case management and implement changes as needed.
Conduct regular performance evaluations and provide ongoing feedback and coaching to case management staff.
Ensure compliance with all legal and regulatory requirements related to case management.
Represent the organization at conferences, meetings, and other events to promote the case management program and share insights and best practices.

Job Qualifications

Minimum Of 10 Years Of Experience In Healthcare, With At Least 5 Years In A Leadership Role In Case Management.
Bachelor's Degree In Nursing, Healthcare Administration, Or A Related Field. Master's Degree Preferred.
Proven Track Record Of Successfully Managing A Large Team Of Case Managers And Achieving Positive Outcomes.
In-Depth Knowledge Of Healthcare Regulations And Reimbursement Systems, Including Medicare, Medicaid, And Commercial Insurance.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate Effectively With Interdisciplinary Teams And Build Relationships With External Stakeholders.
